We are seeking a proactive, highly personable Clinical Intake & Referral Development Specialist with a nursing background to support inbound care inquiries and build referral relationships within the healthcare community

.This role is ideal for someone who can combine clinical understanding, strong communication skills, and relationship-building ability. The right candidate should be comfortable speaking with families, understanding care needs, coordinating next steps, and developing trust with healthcare referral partners such as hospitals, clinics, social workers, senior living facilities, and discharge planning teams
