**Description:**
Covers inpatient or outpatient positions as need to cover for PTO and FMLA coverage. Assess the nutrition status of assigned patients and implement all components of the nutrition care process, according to food and nutrition department policies and local regulatory standards (IDPH, TJC, CMS, etc.). Monitors and evaluates nutrition interventions, makes appropriate adjustments to plans as needed.

+ Assess, interpret, plan, monitor, and document the nutritional status and nutritional needs of patients using evidence-based standards of care, policies and practice guidelines. Perform a nutrition-focused physical exam on patients. Identify and provide age-specific nutrition care and meet the cultural needs of the patients and their families. Work within an inter-disciplinary team to provide a plan of care to achieve the patient’s health goals.
