We are seeking an experienced Chronic Care Management (CCM) Specialist - Patient Ally Proficiency to join a growing healthcare team. This role is ideal for someone with hands-on U.S. CCM experience who is passionate about supporting patients through ongoing care coordination and proactive follow-up.

As a CCM Specialist, you will play a key role in improving patient outcomes by conducting routine follow-up calls, documenting patient interactions, coordinating care activities, and maintaining accurate patient records.
