CHC Nurse Assessor - Band 08b

Role Overview

The successful candidate will undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs. This will include face-to-face assessments across the Birmingham and Solihull areas, with report writing completed either from home or the office.

Key Responsibilities

* Complete CHC and FNC assessments for adults with complex health needs

* Undertake face-to-face assessments within Birmingham and Solihull

* Complete DSTs, Checklists, Fast Track assessments and reviews

* Work with patients, families, carers, advocates and multidisciplinary teams

* Prepare clear, accurate and comprehensive assessment reports

* Support CHC review panels and funding decision-making processes

* Manage a complex clinical caseload independently

* Liaise with care homes, local authorities, acute trusts and community services
