• Provides hands-on care and performs tasks following the Aide Assignment Sheet/Plan of Care.
• Responsible for client care monitoring and communication with other appropriate team members and supervisor(s).
• Responsible for following agency policy regarding procedures or duties that an aide may perform.
• Follows the care plan which specifies the level of assistance required by the client for each ADL task.
• If the client's plan of care requires the in-home aide to provide extensive assistance, the in-home aide shall be listed on the Nurse Aide Registry.
