As a Case Manager, you will be going into member's homes and performing the ATC monitoring visits. This role includes resolving conflicts, reassigning caregivers as needed, training caregivers as needed, handling client/caregiver inquiries, communicating with case managers, and any other field work that is needed.

Responsibilities:

• Preform ATC visits with members according to AHCCCS regulations.
• Respond to phone, email, and text inquiries in a timely and efficient manner.
