Case Manager

Description We are looking for a compassionate and organized Case Manager to support formerly homeless adults living in supportive housing in San Francisco, California. This is a Contract position focused on helping residents maintain stable housing, access essential services, and improve overall well-being through consistent, person-centered support. The ideal candidate will be comfortable managing a high-volume caseload, coordinating with property and service teams, and responding calmly to urgent tenant needs.

Responsibilities:
• Oversee a sizable portfolio of residents, providing ongoing case management tailored to individual housing stability and wellness goals.
• Meet with tenants regularly through outreach, intake follow-ups, and scheduled check-ins to assess needs and strengthen engagement.
