Contribute to maternal health with Sanctum Care Group as a Case Manager for the Prenatal Outreach Resource Team (PORT) in Prince Albert. Deliver collaborative support services for expectant women in need.
The PORT Case Manager will focus on enhancing the health and stability of at-risk expectant mothers in the community. You will coordinate psychosocial support and facilitate access to various health services. Successful candidates are compassionate communicators who excel in problem-solving and relationship-building.
Key Responsibilities:
