We are looking for an experienced Case Manager to join our team at our Intensive Inpatient Program in Ennis! Job Summary The Case Manager provides critical support in coordinating care, discharge planning, and external referrals for clients at Discovery MSO. Working as part of a multidisciplinary team, the Case Manager serves as a liaison between the clinical team, families, and outside providers to ensure seamless and effective continuity of care during and after treatment. Essential Duties and Responsibilities Serve as the primary contact for families and support systems regarding discharge planning and treatment progress. Coordinate outpatient referrals for therapy, psychiatry, primary care, support groups, and housing as applicable. Develop individualized discharge and aftercare plans based on client needs and clinical recommendations.
