Description

Comunilife Inc. is looking to fill a Case Manager position for its Woodhull I Residence, Brooklyn location. The Case Manager’s main responsibility is to provide comprehensive case management services to assigned clients and their family members. The Case Manager is responsible for full coordination of services for all clients assigned; ensure that all assessments and integrated Individual Recovery Plans are completed and all required/needed services are rendered or arranged based on integrated Individual Recovery Plans' specifications. Responsible for a minimum of a monthly home visit.
