Case Manager

Overview:
We are seeking a compassionate and detail-oriented Bilingual Social Services Coordinator to support older adult clients and their families. This role involves conducting assessments, coordinating services, and helping clients access essential benefits and resources.
Key Responsibilities

+ Conduct in-home and in-office assessments to evaluate social and emotional needs of older adults and their families

+ Develop and implement individualized case plans to support client needs

+ Assist clients with applying for benefits and entitlements within the healthcare system

+ Identify and escalate crisis situations or cases requiring additional support

+ Coordinate daily client activities and assist in organizing monthly events

+ Maintain accurate documentation and support ongoing case management efforts
