Case Manager

Position Overview Case Manager/Wraparound Facilitator at Home Base, Inc. is responsible for providing wraparound services to families, coordinating comprehensive support, and ensuring optimal outcomes for youth and families in the behavioral health program. Primary Responsibilities Provide wraparound services to families with a limit of up to 15 members. Coordinate a comprehensive set of supports, resources, and strategies for each member and family. Utilize a strengths-based approach to plan, coordinate, collaborate, and facilitate team meetings with the youth, family, and team members. Ensure services and clinical treatment modalities augment each other for optimal outcomes. Work with the MCO care manager and family to identify natural supports and other community resources.
