We offer hands-on help to address chronic homelessness, mental health, substance abuse, disability, and systematic poverty by offering innovative solutions and proactive services to the most vulnerable in Schenectady County.

Our dedicated  Case Management team  is trained to treat individuals with dignity and respect.

A thorough intake is completed to assess immediate needs, eligibility for programs, and connection to area service providers.

Through consistent follow-up, fostering and cultivating trust, and developing relationships, our clients receive the support network they need to better their livelihood.
