Hours: Week 1: Tues 9am-10pm, Thursday 7am-8pm, Friday 7am-8pm.

Week 2: Tues 7am-8pm, Weds 7am-8pm, Friday 7am-8pm.

Week 3: Tues 9am-10pm, Thursday 7am-8pm, Sunday 7am-8pm.

Week 4: Weds 7am-8pm, Saturday 7am-8pm, Sunday 7am-8pm.

The purpose of the role is to provide autonomous assistance to the People We Support to fulfil their potential for independence, choice, inclusion, and control.

You will be responsible for ensuring that care and support is of a high standard, focused on person centred outcomes and delivered within budget.

You will ensure that all needs assessments, support plans and risk assessments are kept up to date.
