We are recruiting a Care Partner to assess, plan and coordinate care for clients across the Illawarra region. This is a hands-on coordination role where your clinical and case management skills will directly influence the quality of life of the people we serve. 

• Payrate: $44.58 - $47.97/hour + NFP Salary Packaging
• Hours per week: Full time, 38 hours/ week. We are open to a part-time role for the right candidate.
