Care Partner Commonwealth Home Support Program

Resthaven Murray Mallee Community Services are looking for a Care Partner to provide comprehensive coordination and individualized case management and planning in partnership with Commonwealth Home Support Program (CHSP) individuals.

The Care Partner will oversee and assess the services provided, monitoring outcomes against an individual’s service plan, with an aim to optimize a person’s quality of life and achievement of goals. The Care Partner will work with the intake team to onboard individuals to the appropriate CHSP services, ensuring that approved services are provided as outlined within their Support Plan.

This role is offered on a full-time permanent basis, located at Littlehampton (with possible coverage required out of our Murray Bridge office).
