Job Description

Essential
 Criteria
Relevant qualification in Social Work, Allied Health, Community Services, nursing, or another accepted discipline under the Support at Home Manual. Registered or Enrolled Nurses must hold current AHPRA registration.
Willingness to travel for client visits and work from the office as required
Knowledge of Home Care Packages/Support at Home, My Aged Care, and Aged Care Quality Standards
Commitment to helping older people remain independent at home
Strong communication, planning, organisation, and time management skills
Confident using Microsoft Office and client management systems
Ability to build positive relationships with clients, families, colleagues, and service providers
Current and valid First Aid and CPR Certificate
A current Victorian Driver’s Licence
