Care Partner - Aged Care

At Australian Unity, we’ve been walking alongside Aboriginal and Torres Strait Islander communities for over 30 years—supporting Elders to live safely and comfortably on Country with care that honours culture, connection, and community.

As a Care Partner with our Aboriginal Home Health team, you’ll support Elders through their care journey in a way that respects culture and connection to land. Working closely with families, clinical teams and community providers, you’ll develop holistic care plans, coordinate services, and ensure Elders receive safe, person-centred support that enables them to age well at home and on Country.

This is a full-time role based at Albury, NSW working in a hybrid environment, from office/home and completing customer visits. You will required to travel beween Albury and Wagga
