Role Summary
The Care Manager Outreach Services reports to the Program Director and supports Outreach Services at the program office site, providing general advocacy for low-income families in Sullivan County. This role exists to keep the satellite Outreach Office running smoothly and to connect clients to practical resources.Key responsibilities include overseeing daily office operations; maintaining office procedures with Program Director approval; coordinating training with partner agencies (Health Department, Social Services, Mental Health, Adult Protective Services, Child Welfare, etc.); tracking service statistics for CSBG and preparing monthly Board reports; supporting local volunteer boards with training, planning, and elections; drafting press/radio releases for approval; collaborating with community agencies to develop Outreach programs (Food Stamps outreach, SSI outreach, crisis intervention); participating...
