Care Manager

The Care Manager at MD Anderson Cancer Center provides transitions of care through phone and online communication for patients, families, and caregivers following their visit to MD Anderson. This role serves as a frontline clinical resource, helping patients navigate post-discharge needs, close gaps in care, and access appropriate resources while improving patient outcomes, reducing unnecessary readmissions, and enhancing the overall patient experience.
The ideal candidate will have a Bachelor of Science in Nursing (BSN) from an accredited school of nursing, three years of oncology experience in a hospital setting, and an active Texas RN License. Experience in a telephone help line, counseling, teaching, customer service, or population health setting is preferred, and bilingual Spanish/English skills are highly desirable.
