Care Coordinator - Personalised Care

An exciting opportunity has arisen to join our Personalised Care Team within Kirklees.

Are you passionate about working with people and helping them to stay independent and manage their health and wellbeing? If so, Personalised Care is looking to recruit a Care Coordinator to work on a 12‑month fixed‑term project.

Responsibilities

• Work within the Personalised Care Team, a centralised team serving people across Kirklees who live with multiple or long‑term health conditions.
• Hold a 1‑to‑1 caseload and support the implementation and development of Integrated Neighbourhood Meetings across Kirklees.
• Test out the NHS model of proactive working through lone working, home visits and a hybrid approach.
• Demonstrate agreed outputs and outcomes of the project, in line with funder expectations.
