Access Community Health is seeking a proactive Care Coordinator in Nelson to join our regional team. You’ll coordinate rosters, match workers to clients, and ensure timely, high-quality home care empowering people to live independently.
You’ll work closely with clients, whānau, and Support Workers, maintaining records, handling inquiries, and escalating safety concerns when needed. Training and development are provided to support growth.
