Job Description

I am seeking an experienced and compassionate Care Coordinator. This role is responsible for coordinating home support services, ensuring older people receive the right care at the right time, enabling them to remain safely and independently in their own homes.
About the Role
As Care Coordinator, you will work closely with healthcare professionals, care providers, clients and families to assess, arrange and monitor care supports. You will play a key role in case management, service planning, quality assurance and stakeholder engagement.
Grade VII
35 hours per week 
Dun-Laoghaire Based 
Initial 3 month contract with view of extension 
33.64 per hour 
Key Responsibilities
Coordinate and manage home support packages for older people.
Liaise with multidisciplinary teams, healthcare professionals and external service providers.
