Job Summary

Connected Health requires a proactive and dynamic individual to join our Growth and Expansion Team as a Rapid Responder . The role supports the Registered Manager in developing new home care services within the allocated Trust zone.

Key Responsibilities

• Work exclusively with the Registered Manager as part of the Expansion Team
• Support development and mobilisation of new home care services
• Provide cover and support across the allocated Trust zone
• Assist with new care packages, hospital discharges and emergency cover
• Support staff shadowing and ensure Health and Safety policies are followed
• Check and collect daily record sheets from clients' homes
• Deliver care plans and log sheets to clients' homes when required
