Behavioral Health Care Manager (ECM) Licensed (Hybrid) Heritage Health Network (HHN) Join Heritage Health Network as a licensed Behavioral Health Care Manager (BHCM) and support high-need Medi-Cal members through our Enhanced Care Management (ECM) program. This hybrid role includes 23 days in the field and 23 days in the office, allowing for flexibility while maintaining meaningful, in-person member engagement.
