Job description

You will work as part of a multidisciplinary Early Supported Discharge (ESD) team for stroke and neurological rehabilitation, providing high‑quality rehabilitation in community and home settings and supporting patients to regain independence following hospital discharge.

• Deliver delegated rehabilitation programmes under the supervision of registered clinicians.
• Assist patients with functional rehabilitation tasks in their own homes and community settings.
• Help implement physiotherapy, occupational therapy, speech and language therapy, and nursing programmes.
• Undertake basic assessments and contribute to patient progress reviews under supervision.
• Promote independence, safety, and recovery following individual care plans.
• Support group‑based rehabilitation sessions and inpatient/community interventions where required.
