Job Role Summary: 

The Assistant Manager, Dementia Care is responsible for the development, implementation, and evaluation of dementia care programmes and services to support persons living with dementia and their caregivers in the community. The role provides leadership in dementia care practice, staff capability development, programme quality assurance, care planning, and stakeholder engagement to promote person-centred and evidence-informed care. 

You will need to:

1. Dementia Care Assessment and Case Management 
Conduct comprehensive assessments of clients with dementia and cognitive impairment to identify cognitive, physical, functional, psychosocial, behavioural, and social impairment on ADL and IADL engagement for persons with dementia

Formulate a care plan and coordinate care for the individual and their families, identify appropriate providers and facilities throughout the continuum of se...
