We are seeking a highly motivated and results-oriented Project Manager to support the planning, implementation, and ongoing management of Medrina's participation in the ACO REACH/LEAD ACO model focused on high-needs populations across post-acute, long-term care, skilled nursing, assisted living, and community-based settings.
